Welcome to Allergy & Wellness Care! We’re so glad you’re here. To help your first visit go as smoothly as possible, please take a moment to review the important info below. We’re here to make things easy and answer any questions along the way!

Preparing For Your First Visit
Thank you for scheduling your appointment with Allergy and Wellness Care. To ensure we provide you with the most accurate evaluation and best care possible, please follow these guidelines before your visit.
1. Stop antihistamines before testing (if applicable)
If you are scheduled for allergy skin testing, you must stop taking antihistamines for the specified time before your appointment. The following is not a complete list:

Oral antihistamines (e.g., Zyrtec, Claritin, Allegra, Benadryl): Stop 5 days before testing

Antihistamine nasal sprays (e.g., Azelastine, Dymista): Stop 3 days before testing

Antihistamine eye drops (e.g., Pataday, Zaditor): Stop 3 days before testing
-
Certain acid reflux medications (e.g., Pepcid, Zantac): Stop 48 hours before testing

Tri-cyclic anti-depressants and anti-psychotic medications can also act as antihistamines and may interfere with your allergy testing: Do NOT stop antidepressants for any reason without checking with your doctor first
If you have concerns about stopping medication or are unsure if your medication contains antihistamines, please contact our office via the patient portal.
2. Complete your patient forms in advance
To save time at check-in, please complete all required patient forms before your visit. These forms can be found on your patient portal and in text messages sent by our office. Office policy requires you to set up your patient portal.
3. Bring the following items to your appointment

Photo ID, Insurance and Prescription Plan card(s)

Referral (if required by your insurance)

A list of current medications (including prescription, over-the-counter, and supplements)
-
A list of your allergy symptoms and triggers (if known)
-
Please bring a list of any foods you have had a reaction to or a picture of the label

Previous allergy test results or medical records (if available)

4. Expect to be in the office for approximately 2 hours if the doctor orders allergy testing

5. If you are experiencing fever, severe illness, or an active infection, please call our office to reschedule.
6. Do not stop asthma medications (inhalers)
7. If you are pregnant, let us know
Insurance
Benefits Disclaimer
While our office makes every effort to verify coverage and benefits for your services, insurance companies are becoming increasingly less accurate with their benefit estimates. It is our goal to provide you with the best possible care, including cost transparency. However, final insurance determination does not always align with benefits provided by insurance. We do not guarantee benefits.

We strongly recommend that our patients call their insurance plan prior to receiving our services (office visit, testing, immunotherapy, injections etc.) and that a reference number is obtained at the end of the call. While the insurance has the final determination regarding your claim, it can help to have a reference number on file if your insurance company processes the final claim differently than from the benefits provided to you.

These codes may both be covered by your copay, deductible, coinsurance, or one code may be covered by your copay and one by your deductible or coinsurance.
